NEW! EPK & Associates now offers an electronic option for submitting your Employee Waiver of Coverage Forms. The new process is simple and secure for you and your employees.

Here’s how it works:

  1. You send the link to any employee who is waiving coverage: BIAW E-Waiver
  2. Employee clicks link and completes the online form.
  3. Once the employee clicks “Submit” the form is sent to EPK & Associates for review.
  4. EPK & Associates processes the Employee Waiver of Coverage Form.
  5. You receive an email confirming that the e-waiver has been processed.

If you or your employees would prefer to complete a paper version of the form, that option is still available. Please have your employee complete and sign the paper Employee Waiver of Coverage Form and fax it to EPK & Associates.
